CALL OUR OFFICE
Call (239) 200-7248 for an initial assessment/consultation.
*Because we have a very busy office, a deposit will be requested to reserve your spot for an appointment time. It will be refunded to you if we see that you will not benefit from Virtual Health Assessments appointments.
COMPLETE YOUR INTAKE FORMS
Once complete, we will email them to you as soon as your appointment is booked. It is IMPERATIVE that they are filled out before your appointment and emailed back to us. We will not be able to continue with the Virtual Health Assessment without these.
CHECK YOUR EMAIL
A link to a Zoom conference call will be sent to your email once the intake forms are received. You can use Zoom through a Desktop, Laptop, Smart Phone, or Ipad. Make sure that there's a camera and high-speed internet available. If you are using a computer, make sure that you are using Chrome, Safari, or Firefox for your browser.
PREPARE FOR ASSESSMENT
Have a clean area with some space to move around. Try to keep your space quiet and free of distractions.
LOGIN TO ZOOM
Login to the meeting room a few minutes early and make sure that your audio and video are working beforehand! Then Breakthrough Health Center - Bonita Springs will join the Zoom call at the requested appointment time.
VIRTUAL HEALTH ASSESSMENT
During your Virtual Health Assessment, we will go through a consultation and health screening and we will send you recommendations to you via the email you provided for follow-up. Your schedule of visits will be dependent upon the goals you set and the frequency necessary to hit those goals.
